Follicular Unit Extraction: Anatomic and Mechanical Technology
Follicular Unit Excision (FUE) is “incision” made around each follicular unit with a punching procedure to separates it from the tissue around it. The isolated units are then “extracted” from the scalp. Once removed from the donor area the follicular unit grafts are placed into the recipient region of the scalp. Thus, Follicular Unit “Excision” has two components, incision and extraction. The incision part of the procedure is what classifies FUE as a surgical procedure.

The reason for clarifying FUE as surgical graft removal by excisions of hair and tissue is to make this term more scientifically, clinically and surgically accurate said Ricardo Mejia, MD, ISHRS Board Member and Chair of the Ad Hoc Committee on Issues Pertaining to the Unlicensed Practice of Medicine. Performed by qualified and properly trained hair restoration surgeons hair transplantation becomes more safe and highly effective procedure to correct hair loss. However, it is a cosmetic surgical procedure and any attempt to downplay the significant surgical aspects of FUE to appear less invasive is misleading, and potentially dangerous, to patients.
However, the world's leading authority on hair loss treatment and restoration, the ISHRS feels a responsibility for consumer education which supports patient safety, promoting truth in advertising, as well as to educate patients about all surgical and medical options available to them, said Dr. Mejia. "Amending FUE terminology is one more way that we can inform patients about the true surgical nature of the procedure so they can make better decisions about their choice of physician and treatment—and avoid inadvertently paying to have surgery performed by someone not licensed to practice medicine."

In the present time the robotic hair transplant technique introduced at Bernstein Medical is pre-making recipient sites. Using this strategy, recipient sites are created before follicular units are extracted in a typical one-day procedure (or at the beginning of the first day in a two-day procedure). This enables the grafts to be placed into recipient sites as soon as harvesting is complete. The technique minimizes the time grafts are outside the body, increases visibility when placing the grafts (due to less bleeding), minimizes graft popping, and allows the recipient area to start the healing process earlier. A better environment for newly transplanted grafts increases the chances that they will survive the transplant and increases the likelihood of an excellent aesthetic outcome.
